St. Francis Gardens

St. Francis Gardens Day Care Centre is nurse-led and provides a warm, safe and welcoming environment for older adults to relax and socialise. The aim of St Francis Gardens is to deliver person centre care and support to you so that we can help enhance your quality of life and independence.Our goal is to ensure your physical and emotional needs are met.

As a nurse led centre we provide health monitoring and assessments in addition to a wide range of social supports and activities. We provide a subsidised transport scheme, via taxi, for those who cannot provide their own transport. Breakfast and lunch is provided daily from our in house kitchen.Referral to our service is made though the PHN, MDT, mental health team, GP or by your geriatrician. You can self-refer once your application is signed by a medical professional.
