La Verna Daycare

La Verna is a nurse-led service catering for those over 65 with diverse needs and conditions. Our service has 2 dementia specific care days. Our staff are fully trained in providing excellent personal care, dementia support, CPR, infection control, and safeguarding of vulnerable adults.Staff provide support and advice for often overwhelmed family carers. We are well placed to recognise emerging conditions or concerns and can trigger early interventions so as to avoid hospitalisations and premature placements in Long Term Care.We liaise with the multidisciplinary healthcare team, organise referrals and advocate on clients behalf to ensure they are receiving the most appropriate care available to support them at home.We provide tea / coffee / snacks and a lunch each day. While here, Service Users can expect chat, games, quizzes, an exercise programme, a 3course lunch, health monitoring and wellness as you age education. Referral to our service is made though the PHN, MDT, mental health team, GP or by your geriatrician
